This framework covers the key elements or best practices of a successful transition program for young adult literacy learners who have exited high school but still need literacy training in order to transition to further educational studies or workplace training. This framework is grounded in a review of the literature, and covers three stages for developing a transition program that will best support learners as they overcome challenges in their attempts to succeed in educational and workplace settings: identify and recruit learners; support and retain learners; teach and transition learners. Each stage is illustrated by real success stories, a list of action items to consider and address, suggestions for building connections with various stakeholders, and a “Tool Box” of samples and practical tools which can be used and modified.
